The Effect of Insight and Executive Function on Treatment Adherence in Patient with Alcohol Dependence

Abstract
Objectives £º Alcohol dependence is a chronic relapsing brain disorder that is characterized by relapses requiring longitudinal long-term management strategies. Thus, treatment adherence and medication compliance in the treatment process of alcohol dependence are very important components. The purpose of this study is to investigate treatment adherence to the outpatient treatment system after inpatient treatment and to figure out the relationship between treatment adherence and insight level as well as the executive function of the frontal lobe.

Methods £º We enrolled 55 patients who had been admitted to an alcohol treatment program and diagnosed with alcohol dependence according to DSM-IV. The subjects were evaluated by sociodemographic and clinical characteristics. After 3 weeks of detoxification, Hanil insight scale and executive intelligence test were administered. After the inpatient treatment programs, we followed up with the patients for 12 months and conducted a checkup at the 1st, 3rd, 6th, and 12nd months in order to observe how many patients were maintained in the outpatient treatment program as well as to obtain the mean cumulative number of patients¡® visits in the outpatients treatment program.

Results £º As expected, it was difficult to maintain patients in the outpatient treatment program for 12 months at every checkup point ; about 50% of the patients dropped out at every checkup point. Even though there was no statistical difference, the fair insight group visited more the outpatient treatment program and dropped out more slowly. When we compared the executive function performance according to either the follow-up or not, the follow-up group began to show a significantly different higher performance of executive function after 6 months.

Conclusion £ºMaintaining the treatment adherence of the outpatient treatment program after the inpatient treatment program is very important. Treatment adherence, particularly in long-term management, is related with insight level and executive function.
